Financial/Billing Information

Children’s Behavioral Medicine Collaborative providers are not contracted with any insurance companies; however, most commercial insurance companies will accept a superbill for reimbursement and a few insurances will approve a benefit exception that reimburses at the in network as opposed to the out of network rate. We encourage families to reach out to their insurance companies to inquire about their reimbursement rates and/or request a benefit exception.

We typically provide a superbill that you may submit to your health insurance to request reimbursement. A superbill includes our practice’s Tax ID number, NPI number, diagnosis(es) codes, and the service/billing codes. Most commercial insurance companies will accept a superbill for reimbursement. However, we cannot guarantee you will receive any reimbursement from your insurance company, and the reimbursement rates will vary depending on your insurance and/or type of plan. We encourage you to reach out to your insurance to inquire about their reimbursement rates.

Below are the procedure/billing codes that the insurance company may ask you about when you inquire about reimbursement rates and/or you request a benefit exception so that you may be reimbursed at the in network as opposed to the out of network rate for their respective services.

These are the billing codes that are typically used as part of the neuropsychological evaluation.

  • Intake or Consultation appointment:
    96116 (first hour) and 96121 (second hour)
  • Testing appointments:
    96136 (first 30 minutes)
    96137 (subsequent 30 minutes - up to 11 units if we need all six hours for testing)
  • Evaluation, feedback session to go over test results and recommendations, interpretation, and report preparation:
    96132 (first hour)
    96133 (subsequent hours - up to six units but will depend on time spent)

These are the billing codes that are typically used for psychotherapy and biofeedback services.

  • Intake or Consultation appointment:
    90791 (intake/consultation visit)
    96116 (first hour) and 96121 (second hour)
  • Follow-up psychotherapy codes (billed by time):
    90837 (60 min visit)
    90834 (45 min visit)
    90832 (30 min visit)

Adoption Support Program (Washington State Department of Children, Youth & Families)

The Adoption Support Program may pay for a portion of a neuropsychological evaluation and for a certain number of psychotherapy visits in a year. It is recommended that you reach out to your DCYF case manager to inquire about your child’s eligibility for the Adoption Support Program.

If you are eligible for the program, you will have to be preauthorized for services by DCYF. This entails filling out the ‘Preauthorization for Services’ form for the Adoption Support Program. Please send us the form after you have completed the top half of the form, as we will have to fill in the practice’s information prior to sending it off to DCYF. It will also be helpful for you to provide us with the email address of your DCYF case manager so that we can email the form directly to your case manager.
